The Capati Difference Wellness Plan gives patients an alternative way to obtain dental treatment for reduced fees.

Quality dental care is an essential part of healthy living and needs to be affordable for our patients, which is why Dr. Albert Capati developed The Capati Difference Dental Wellness Program. It is not dental insurance, nor may it be combined with dental insurance or other discounts; however, it does allow you to obtain dental treatment for reduced fees. Membership plans begin as low as $250 per year per person.

- No pre-authorization requirements & no pre-existing condition limitations
- No denial of insurance coverage
- No waiting periods
- No restrictions on elective or cosmetic procedures
Annual Wellness Program Membership Benefits:
- 2 dental cleanings *
- 2 periodic examinations *
- 2 oral cancer screenings *
- 2 fluoride treatments *
- All necessary X-rays & photographs
- 15% off all treatments; excludes referred treatments to specialists
- 10% off all dental products
- Emergency exam
* If the patient misses a scheduled appointment and does not give 24-hour notice of cancellation, the appointment will count as one used benefit cleaning, periodic exam, oral cancer screening, and fluoride treatment for the plan year. Perio patients will receive the value of one cleaning of the price of one quadrant scaling or periodontal maintenance and the balance will be discounted according to the plan.
Program
Guidelines
- Treatment fees are due the day of service unless a finance plan has been approved.
- Membership fee monthly payments must be set up as automatic withdrawal either through a credit card or bank account.
- The membership annual charge or monthly payments will automatically renew on your Membership Anniversary Date unless we receive prior notice of cancellation from you.
- If payment is discontinued, there is a $50 reinstatement fee.
- This program cannot be used in conjunction with another dental plan, dental insurance, or outside financing program.
- No refunds of membership fees will be given unless requested within 30 days of enrolling. If any benefits have been used within the first 30 days, the discounts will be removed and all treatment will be required to be paid in full.
- No refunds for unused annual membership benefits.
